Simply type your data and observations into the form and e-mail it to the BAP Chairperson.

Staff,

Wouldn't you have to print the file to a PDF output to save the info on the form? When you click on the form, it says that info on the form cannot be saved.

Foxit allows you to edit and save PDF files. Here is a link to the Windows version : http://www.foxitsoft...df/rd_intro.php
